Care Coordinator
Join our team in a pivotal role where you'll use a person-centered approach to coordinate care across various health services. As a Care Coordinator you'll be instrumental in providing access to physical and behavioral health care, social services, housing, education, and employment opportunities, all aimed at fostering wellness and supporting the recovery of the whole person. Your expertise in care coordination will ensure alignment with the preferences and needs of those you serve, staying in line with their current treatment plans. NKHS is a 501(c)(3) private not-for-profit organization operating with the purpose of promoting high quality, comprehensive community mental health programs in Vermont’s beautiful Northeast Kingdom. Our mission is to empower individuals, families, and communities by promoting hope, healing, and support. NKHS is a Designated Agency contracted with the State of Vermont to provide the highest quality of service in the areas of adults with intellectual/developmental disabilities, children and youth with serious emotional disabilities, and adults with mental health and substance use challenges. We serve the populations of Caledonia, Essex, and Orleans counties through a wide variety of targeted programs for individuals of all ages. NKHS has over 450 employees providing case management, community and home supports, residential care, psychiatry, medication management, therapy, vocational supports, school based counseling, emergency care, and respite services. RESPONSIBILITIES & SCOPE
  • Promote community integration of individuals and assist/encourage natural supports necessary.
  • Counsel clients about the use of the National Suicide and Crisis Lifeline, 988, local hotlines, warm lines, mobile crisis, and other needed resources.
  • Assess clients need/desire in accessing benefits, such as Medicaid and financial assistance.
  • Develop partnerships with a variety of community services and supports.
  • Contact any clients that have been discharged from an inpatient setting within 24 hours of discharge to connect them to services.
  • Provide service coordination with a medical focus among internal and external providers.
  • Reviews schedule daily and contacts clients that no show and reschedule immediately. Discusses barriers to attending appointments.
QUALIFICATIONS
  • High school diploma or equivalent.
  • At least 2 years’ experience in a behavioral health or primary care office setting

  • Competitive Salary: $18.25/hr based on education and experience.
